Patients' Rights And Responsibilities
Patients should attend their appointments at the arranged time. If you are unable to attend an appointment, please help to reduce waiting times by contacting the surgery and cancelling it as soon as
possible. When an appointment is not kept it will be noted in your medical records.
All services will be provided professionally and courteously and you, the patient, should reciprocate this level of behaviour.
Patients have the right to confidentiality and doctors, nurses and all other members of staff operate a strict policy of patient confidentiality.
Patients are welcome to view their medical records. Please ask at reception for an application form to arrange this.
It is our job to give you treatment and advice. Following discussion with you, you will receive the most appropriate care, given by suitably qualified people. No care or treatment will be given without your
informed consent. In the interest of your health it is important for you to understand all the information given to you. Please ask questions if you are unsure of anything.
Confidentiality
We ask you for personal information so that you can receive appropriate care and treatment. This information is recorded on computer and we are registered under the Data Protection Act. The practice will ensure confidentiality is maintained at all times by all members of the practice team. It is, however, necessary for the effective functioning of a multi-disciplinary team that medical information is sometimes shared between members of the practice team.
